What foreigners actually pay at Bali’s hospitals and clinics, the three price tiers, and how to tell before you are admitted whether your insurer will settle directly or leave you to claim it back.
Expect to pay two to four times the local rate at an international hospital, and roughly 50–70% less than Australia, the UK or the United States for dental work and elective surgery. The gap narrows for complex surgery once you count the follow-up you will need at home.
The single biggest variable is not the procedure — it is whether the hospital bills your insurer directly.
The same treatment can differ threefold across these. Which tier you are in is decided when you choose the facility, not at the till.

Highest price · Lowest friction

Often the same standard of clinical care for routine work at a fraction of the price. English varies, and you will usually pay and claim back.
Best value for routine care

The lowest cost by a wide margin and the deepest capability for serious trauma. Longer waits, more variable English, less comfort.
Lowest price · Where critical cases go
International-patient rates at private facilities, paid out of pocket. Public hospital prices are materially lower.

| MRI scan | Rp 2,500,000 – 6,000,000 | With contrast costs more |
| Medical evacuation to Singapore | USD 15,000 – 40,000+ | Get insurer approval in writing first |
Indicative ranges compiled from published international-patient rates, not quotations. Prices move with the exchange rate and are renegotiated annually — confirm in writing with the facility before you commit.
Four things that decide whether you are reimbursed or out of pocket.
Most standard travel policies cover emergencies only. Elective treatment you flew in for is usually excluded outright, whatever the clinic tells you.
Cashless billing means the hospital settles with the insurer; without it you pay the full amount and claim later. Ask the hospital and the insurer, not just one of them.
Cashless billing explained — Confirm the hospital bills your insurer directlyFor anything planned, and as early as possible for anything urgent. Verbal approval on a phone call is not something you can produce during a dispute.
Ask at the time of payment. An itemised English invoice obtained months later, if at all, is the most common reason a valid claim fails.
For dental work and elective cosmetic procedures, usually substantially — commonly 50–70% less than Australia or the US. For complex surgery the saving shrinks once you count flights, recovery time and follow-up at home.
Facility tier, materials, and whether the surgeon is a senior specialist or a junior one. A dental implant price that undercuts the market usually reflects the implant brand, not efficiency.
For planned treatment, usually a deposit up front. For emergencies, you are stabilised first and billed afterwards — which is exactly why engaging your insurer early matters.
The international and larger private hospitals do, with limits. Local clinics often prefer cash. Card limits catch people out on larger bills, so check yours before you travel.
Rarely. A surgical quote typically covers the procedure, not complications, extra nights, or medication on discharge. Ask specifically what is excluded.
